WholeYouNYC connects Medicaid members in New York City to vital services beyond healthcare— like food assistance, housing support, and transportation help. The Juan Pablo Duarte Foundation is now a proud partner of WholeYouNYC, and joins a network of 300+ community organizations that make it easy to find and access the benefits you may already qualify for. Starting this Monday, March 9th, our Community Center will have a team of screeners happy to check your eligibility and connect you to free services that support the overall well-being of our community. Learn more about it below.

Medicaid members can also use a quick self-screener by clicking here. And call this number to ask for assistance: 888-755-5045. The process is free, confidential, and available in multiple languages including Spanish.

As reinforced by Laura Acosta, Founder and Executive Director of the JPD Foundation, JPD’s goal has always been to build a healthier and stronger Uptown Manhattan. “This new partnership is designed to bridge the gap between community members and essential health services. By bringing resources directly to the neighborhood, we aim to ensure that every resident has the information they need to lead a fair and vibrant life. Our bilingual team will be out and about talking to different members of our community to make sure that they get the most out of their Medicaid. Many people believe that Medicaid is just for older people, but it is not. We want to make that clear too”, concluded Acosta.
